Health Net Federal Services, LLC (HNFS) is committed to maintaining the privacy of those who communicate with us through our website (the site).

We have created this Web Privacy Policy in order to demonstrate our firm commitment to your privacy and to make clear what happens to any information we collect. This Policy also discloses our information gathering and dissemination practices for the site.

Occasionally, the terms of this Policy may change. We reserve the right to change this Policy without notice to our users.

Privacy Act Statement

This statement serves to inform you of the purpose for collecting personal information required by TRICARE and HNFS and how it will be used.

AUTHORITY: 10 U.S.C. Chapter 55; 38 U.S.C. Chapter 17; 32 CFR Part 199; 45 CFR Parts 160 and 164, Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security Rules; and E.O. 9397 (SSN), as amended.

PURPOSE: To obtain information from individuals necessary for their enrollment in TRICARE Programs including managing enrollment through web-based tools, assisting individuals in obtaining authorizations, eligibility determinations, healthcare provider referrals, and customer services, and facilitating medical management, provider services, and payment activities.

ROUTINE USES:  In addition to those disclosures generally permitted under 5 U.S.C. 552a(b) of the Privacy Act of 1974, as amended, these records may be specifically disclosed outside the Department of Defense as a routine use under 5 U.S.C. 552a(b)(3) as follows: to the Departments of Health and Human Services and Homeland Security, and to other Federal, State, local and foreign government agencies, private business entities under contract with the Department of Defense, and individual providers of care, on matters relating to eligibility, claims pricing and payment, fraud, program abuse, utilization review, quality assurance, peer review, program integrity, third-party liability, coordination of benefits, and civil and criminal litigation.

DISCLOSURE: Voluntary. If you choose not to provide your information, no penalty may be imposed, but absence of the requested information may result in administrative delays or the inability to process an individual’s request.

Your Personal Information

Health Net Federal Services holds in the highest regard privacy and data security. Health Net Federal Services discloses an individual's protected health information only as permitted or required by applicable federal privacy regulations and state laws, consistent with Covered Entity's Notice of Privacy Practices.

Security

Our site requires users to give us unique identifiers in order to log in to many areas of the site. These identifiers include, as appropriate, the subscriber identification number, provider identification number, and/or an assigned personal identification number (PIN). We utilize these unique identifiers to verify the user's identity and eligibility, in order to protect our members from the release of sensitive or personally identifiable information to unauthorized users.

You must take responsible and appropriate steps to safeguard your identification numbers to help protect your privacy.

Our site has additional security measures in place to protect against the loss, misuse and/or unauthorized alteration of the information under our control, including but not limited to:

  • Built-in firewalls (a combination of computer hardware and software) to keep unauthorized users from accessing information through HNFS' computer network.
     
  • Encryption technology, including Secure Sockets Layer (SSL) for certain communications.

We aim to protect and keep confidential all information which is voluntarily provided to HNFS through this site, and to treat such information with the same consideration and confidentiality as any information sent to us by the U.S. mail or communicated to us by telephone.

Third Party Content Providers and Links

Users will find content and products of other companies or organizations on our sites and/or links to the sites and services of our suppliers, licensors or other third parties. While we confirm working links, HNFS does not control the contents that appear on those sites. We encourage you to review the privacy policies of any third party sites or services before providing any of them with your personal information.

Outside Parties

We have hired outside vendors and contractors to assist us with the development, improvement and maintenance of this site. We have taken measures to ensure that any personally identifiable information given to us remains with HNFS and its subcontractors for lawful purposes and is not passed through to outside vendors or contractors, unless the user voluntarily gives, or authorizes us to give, the vendor that information.

Information Gathering

Personally Identifiable Information
Personally identifiable information means data unique to the user, such as a name, address, email address, Social Security number, date of birth, or telephone number. Health Net Federal Services will not collect personally identifiable information about you unless you or your representative provides us that information voluntarily. Health Net Federal Services may collect personally identifiable information about you from you or your representative on applications and order forms, through requests for information, products or services, or in order to fulfill orders, deliver requested materials and information about HNFS to you, respond to your questions, or deliver a product or service. We also use your contact information to get in touch with you when necessary.

If you provide personally identifiable information to us voluntarily through our site, we will not sell, license or transmit that information outside HNFS and those affiliated companies involved in administering or providing services for our health benefits plans, unless you or your representative expressly authorize us to do so.

We will disclose personally identifiable information in compliance with applicable laws and regulations and will require the recipient to protect the information and use it only for the purpose provided.

Internet Protocol Addresses

An Internet Protocol address is a number that automatically identifies the computer/machine you have used to access the Internet, the way a phone number identifies you on the phone system. The address enables our server to send you the webpages that you want to visit, and it may disclose the server owned by your Internet Service Provider. We utilize your Internet Protocol address to help diagnose problems with our server and to administer our site.

Cookies

Cookies are pieces of information that a website transfers to a user's computer for purposes of storing information about a user's preferences. Just as many major websites use cookies as a standard practice to provide useful features when a user visits the site, we use cookies to deliver content specific to your interests. Cookies in and of themselves do not personally identify users, although they do identify a user's computer. A user can set his or her browser to refuse cookies, but then he or she may not be able to take full advantage of the site.

General Information

Health Net Federal Services routinely gathers quantitative information on website activity, including the number of users to the site, and the pages visited. No personally identifiable information is collected in this routine activity, and HNFS reserves the right to share such information with others.

Do Not Track Signals

Health Net Federal Services does not currently honor a user's request to opt out of the collection of personally identifiable information about an individual consumer's online activities over time or across third-party websites or online services when the user makes such a request using a Web browser via its Do Not Track option or other mechanism consisting of a Do Not Track signal. If you have your browser set to private browsing, you may not be able to take full advantage of the site.
